Insurance and Billing

Our clinic strives to ensure that those who need a prosthetic eye have options.

We accept all major credit cards, payment plans through Cherry, and Care Credit.
Please review insurance information below and contact us with any questions. 

Insurance 

Artificial Eyes are typically covered by insurance as a medical necessity to support the continued health of your eye socket. All patients using insurance will need to have a referral prescription from their doctor.

Prosthetic eye services fall under Durable Medical Equipment (DME) within your medical coverage, not your vision plan.

Most plans have guidelines regarding replacements and benefits vary. 

We are here to support and assist as we are able, but it is the patient's financial interest and responsibility to ensure that the referrals and prior authorization are confirmed prior to the appointment.

Private Insurance

We are in network with Blue Cross Blue Shield (excluding HMOs) as well as some Aetna plans. 

 

United Healthcare (UHC) and UMR are not in our network. Authorization will need to be requested in advance of the appointment. Payment will be due at the time of service. We will provide you information to include with your claim reimbursement request. 

Medicare

We are a non-participating provider for Medicare and provide courtesy billing.

Payment is due at the time of service and we will submit the claim for you to be reimbursed.*

Medicare will pay 80% of their allowable. If you have supplemental coverage, they typically cover the remaining 20% of the allowed amount. 

Medicare Advantage Plans

Similar to standard Medicare patients, we collect payment at the time of service and courtesy bill for you.

Out of network authorization is usually required prior to the appointment is required to ensure that the claim is approved. If you need assistance, please contact us. 

Medicaid

We accept Medicaid claims as assigned, but prior authorization is required.
